Would you like to get a copy of your medical records from our office?  What you need to do is fill out a "Records Release Form" with a copy of your driver's license and we will get those records for you.  You can download the form in the "FORMS" page on our website, call our office to have us mail you one or stop in the office at your convenience.

After you fill out the form and return it to our office, it will take (3) business days and we will have them ready for you.  You can pick the copies of your records up or we can fax them to you and/or your choice of provider at NO charge.  If you want the records mailed to you or another facility, it will add additional time for your copies to be delivered.
